Back to School Voucher

Students please fill up the form for registration of voucher.
Terms and conditions:

1. This voucher is given to the first 20 registration only.
2. This voucher is strictly given for the following items sales. (1 pair of uniform set, 1 pair of shoes and socks.) This voucher is non-refundable and cannot be exchanged for cash in part or full and is valid for a single transaction only.
3. The closing dateline for this registration voucher is on the 17 January 2020.
4. This voucher is strictly for families who earn below than RM 2,000 in ONE family household.
5. Only maximum of 2 children per family is allow to register for this voucher.
6. This voucher can be collected at the NBVM parish office, from Monday to Friday, 9am – 5pm. For any information please contact 012-4353292 (NBVM parish office).
